The Report type Type will not include Scorecard Category Score % field and Category Sort Order field to order the categories in the report based on the template ordering.
In this case these 2 fields need to be added to this report type via lookup.

Find the fields in the Scorecard Question section and rename them for Category Score % and Category Sort Order
How to rename the fields: double clic click on the field > rename > OkOK
